By submitting New Student registration form, I acknowledge that I have read, understood, and agreed to the Liability Waivers, Codes of Conduct, Rules, and Policies of Crystal School Rhythmic Gymnastics. I commit to adhering to these guidelines and supporting my child in doing the same.

By submitting new student registration form, you accept the following:

**Liability Waiver:** I/We, the undersigned, acknowledge the risks involved in rhythmic gymnastics and agree to hold Crystal School Rhythmic Gymnastics harmless for any injury that may occur during activities.

**Media Release:** I/We hereby give permission for images of my/our child, captured during regular and special activities through video, photo, and digital camera, to be used solely for the purposes of Crystal School promotional material and publications, and waive any rights of compensation or ownership thereto.

**Medical Emergency Authorization:** I/We authorize Crystal School Rhythmic Gymnastics and its staff to obtain medical care for my/our child in case of emergency, if I/we cannot be reached or if the severity of the situation requires immediate attention.

Codes of Conduct Rules and Policies will be included to ensure a clear understanding of the expectations and requirements for the students and their families:

Enrollment and Class Placement

  • Enrollment Fee: There is a non-refundable enrollment fee of $50 for all new students.
  • Placement: Based on the evaluation of skill level by our coaches, students will be placed in the appropriate class to ensure optimal learning and safety.

Dress Code and Appearance

  • Uniform: It is mandatory for gymnasts to wear the school uniform at every class. The uniform has been designed considering the safety and requirements of rhythmic gymnastics.
  • Toe Shoes: Gymnasts must wear appropriate toe shoes during all practices. Barefoot or socks are strictly not allowed.
  • Hair: Hair must be securely tied up in a high ponytail or a bun to prevent any accidents or obstructions during practice.
  • Jewelry: For safety, no jewelry is allowed except for small earrings.

Attendance and Payments

  • Missed Classes: There are no make-up classes for missed practices. We do not offer refunds or reduce payment for sessions that the student has missed.
  • Payment Schedule: Payment for classes is due every month from the 1st to the 5th. A $50 late fee will be added to the payment for any payments made after the 5th.
  • Refunds: If payment has been made for the whole month and the student has attended at least one practice, we will not issue a refund for the class.
